Not everyone has a doula at their birth - and not everyone needs one. But everyone needs a birth partner: the person standing closest, holding a hand, making eye contact through a contraction.
Your birth partner is usually your life partner - boyfriend, girlfriend, fiancé, spouse. Sometimes it's a close friend or family member instead. Whoever it is, they share a deep emotional bond with you, and that's exactly why their presence matters so much. This role can't be outsourced. It has to be them, showing up fully.
Here's the good news: research backs this up. A Cochrane review (one of the most trusted sources for evidence) found that continuous labor support (from a partner, family member, friend, or doula) is linked to more spontaneous vaginal births, shorter labors, less need for pain medication, fewer cesareans, and higher satisfaction with the birth experience overall. No harms were identified. In other words: showing up matters, and it's backed by data, not just tradition.
So why do so many partners feel lost?
Most partners genuinely want to help - they just don't know how. They may feel unsure about bodily fluids, uneasy watching someone they love in pain, or simply unclear on what their job even is.
Here's the secret: the best partners aren't the ones who know exactly what to do. They're the ones who ask.
Ask her how she wants to feel during birth. Then ask how she wants you to help her get there. This looks completely different from couple to couple, and there's no universal right answer.
I've been at births where the partner never left the couch, and births where they caught their own baby. Both were exactly right - for that couple. The difference isn't what they did. It's that they'd talked about it beforehand.
Advice for your birth partner:
- Talk about it before labor starts. Discuss hopes, fears, and expectations well before contractions begin - not in the middle of them.
- Get educated together. Take a class, read a book, watch a video. Birth prep shouldn't fall entirely on the person giving birth.
- Learn a few go-to comfort positions. Simple things - hip squeezes, counter-pressure, position changes - can make a real physical difference.
- Show up and hold space. You don't need the perfect words. Steady presence is often the whole job.
At the end of the day, your role isn't to fix, manage, or perform. It's to witness and support, with your whole self, as she does one of the hardest, most powerful things she'll ever do.
